Prize Pickup Instructions
Did you win a prize on the My SiSStaS Keeper Radio Show? If so, you may pick up at designated location. (Please wait until noon on the following business day to come and claim your prize.
To be fair to all our listeners, we here are our policies for our prize winners:
1. One winner per household every 30 days. If prize exceeds $100, time increases to 90 days. Household restriction includes anyone living in the same house including but not limited to spouse, children, parents, significant other and/or roommate.
2. You are responsible for any taxes and you release My SiSStaS Keeper from all liability arising from prize dysfunction or expiration. All winners are required to complete a W-9 tax form
3. Prizes not claimed within 30 days of winning date will not be awarded. No exceptions.
4. Winners must be have a valid picture id and must be at least 16 years of age.
5. We will only mail prizes to those winner who live more than 35 miles from the radio station.
6. If you will have an representative pick up your prize, you must email us at mysisstaskeeper@gmail.com to arrange such the pick up. Please make sure they have a valid picture ID.
